
Most people step into the shower expecting a clean start to the day. The reality falls short of that expectation.
The showerhead spraying water onto their face has likely never been cleaned on the inside—not by them, not by the previous resident. Warm, wet, and dark, it provides an ideal environment for bacteria to grow. Every time the water runs, some of that bacteria escapes in a fine mist, invisible but inhaled.
The one fixture nobody cleans
Few people remember the last time they scrubbed the inside of their showerhead. Faucets and grout receive regular attention. Toilets get scrubbed. The showerhead, however, often goes untouched for years.
The CDC recommends cleaning showerheads whenever visible gunk appears. Most ignore this advice.
What’s growing inside
Inside an uncleaned showerhead, a thin, slimy layer called biofilm forms. This layer consists of bacteria embedded in a protective slime, clinging to the interior. The slime shields the bacteria from chlorine in tap water, allowing it to thrive. The CDC describes it as “pipe slime,” an accurate description if you unscrew an old showerhead and look inside.
Scientists at the University of Colorado Boulder conducted the largest study on showerhead bacteria, testing 656 units across the U.S. and Europe. The most common bacteria found was nontuberculous mycobacteria (NTM), a group of germs that can cause lung infections. In some homes, NTM accounted for over 99% of the bacteria inside the showerhead. Their study, published in mBio in 2018, also found one type of NTM, Mycobacterium avium, at levels 100 times higher in showerhead gunk than in the incoming water.

The CDC lists showerheads as a breeding ground for three other dangerous germs: Legionella, Pseudomonas aeruginosa, and Naegleria fowleri. The mist from a shower spreads these bacteria. When water hits the body or the shower floor, tiny droplets break off and float in the steam. Those droplets are inhaled without notice. The agency confirms that breathing in these germs poses the primary risk.
The Colorado study mapped where the germiest showerheads were in the U.S. Those regions had the highest rates of NTM lung infections. Most healthy adults can shower under a contaminated head for years without getting sick. The CDC states the body usually handles it. But for some, the risk is serious.
People who should take extra care include smokers, former smokers, and those with chronic lung conditions like emphysema or cystic fibrosis. Anyone with a weakened immune system—due to cancer treatment, organ transplants, HIV, diabetes, or serious kidney or liver problems—also faces higher risks. For these groups, cleaning the showerhead becomes a necessary task. Even for others, the crusty buildup inside clogs spray nozzles, reducing water pressure or causing uneven spray.
Why a new showerhead won’t fix it
Many assume buying a new showerhead solves the problem. It doesn’t. The bacteria live in the water pipes feeding it, not the head itself. The white crust on the outside is mineral buildup, left behind when water evaporates. That crust gives the slime a surface to cling to, but the source remains the pipes. A new showerhead will start collecting the same buildup the same way.
Replacing the head every year won’t solve the issue. A simple, inexpensive method will.
The simple solution
The answer is white vinegar. A single bottle costs one to three dollars at any grocery store, and many already have one in the pantry. The CDC officially recommends soaking a showerhead in white vinegar to remove buildup, as noted on its page about preventing waterborne germs at home.

Vinegar works because the crust the bacteria cling to is mineral buildup. Vinegar, a mild acid, dissolves those minerals. Without the crust, the slime has nothing to stick to. The bacteria wash away with the vinegar.
Here’s how to do it: Grab a zip-top plastic bag large enough to fit over the showerhead’s spray face. Fill it halfway with plain white vinegar—no additives like baking soda, bleach, or dish soap. Tie the bag around the head so the spray holes are submerged. A rubber band or hair tie works well. Leave it overnight, six to twelve hours. Twelve hours if the head is heavily crusted.
In the morning, remove the bag and pour the vinegar down the drain. Turn the shower on hot and let it run for two minutes to flush out any loosened debris. The entire process takes about two minutes of hands-on work.
For showerheads with brass, gold, or nickel finishes, vinegar can dull the coating. In those cases, use a citric acid cleaner instead, following guidance from home improvement sources. If the head unscrews easily, drop it in a bowl of vinegar instead of using the bag.
This method isn’t permanent. As long as the showerhead is connected to the pipes, the slime will return. The Water Quality and Health Council calls any cleaning temporary. For homes with hard water—about 85% of American households—cleaning should happen once a month. For homes with soft water, every two to three months is sufficient.
